What is IVIG?

IVIG (intravenous immunoglobulin) is a blood product derived from the plasma of healthy donors, containing antibodies that can modulate the body’s immune response. It works by introducing a high concentration of donor antibodies into the bloodstream to help neutralize the abnormal antibodies that are characteristic of myasthenia gravis.

For MG patients, the body’s immune system attacks components of the neuromuscular junction, such as acetylcholine receptors or proteins like muscle-specific kinase (MuSK). This interferes with normal neuromuscular transmission, leading to symptoms like muscle weakness, difficulty breathing, and muscle fatigue.

While the exact mechanism by which IVIG works is not completely understood, clinical research suggests multiple pathways, including:

  • Neutralizing harmful antibodies that attack the neuromuscular junction.
  • Altering antibody production by suppressing the immune system.
  • Reducing inflammation caused by an overactive immune response.

These combined effects make IVIG treatment an excellent option for both managing acute exacerbations and providing maintenance therapy in myasthenia gravis.

How IVIG Helps in Myasthenia Gravis

In MG, the immune system interferes with signals sent to skeletal muscles, causing symptoms like muscle weakness, fatigability, and impaired vital capacity. During an MG exacerbation, a sudden worsening of symptoms, IVIG helps by:

  • Blocking autoantibodies that disrupt acetylcholine transmission

What to Expect During an IVIG Infusion

  • Risk of blood clots, particularly in high-risk individuals

Is IVIG Right for You?

  • As a short-term therapy before surgery or during crisis episodes

For patients with frequent relapses or severe symptoms, IVIG may be used periodically, depending on their neurologist’s clinical judgment. Discussing quantitative myasthenia gravis scores, medical history, and previous response to therapy with your care team is essential.
